INTAKE FORM:
- Depending on the age of the patient, please fill out either the adult form or the pediatric form.
CONSENT FORM:
- Please read it carefully and sign it if you agree with what it says. If there are any questions remaining, wait until the visit, when we will be addressing all your questions, so you will feel comfortable with signing your informed consent.
